Service coordinators provide resources, referrals, and light case management to senior and disabled clients. This includes assistance with medical coverage, home care assistance, transportation, and many other services. The service coordinators are assigned to GJHA’s four senior/disabled properties. Additionally, a partnership with Rocky Mountain Health plans allows service coordinators to proved health focused care coordination services to clients receiving Medicaid.
Care coordinators also assist Dual Eligible Special Needs Plans (D-SNPs) clients, who are beneficiaries eligible for both Medicare and Medicaid. These are Medicare Advantage health plans that offer specialized care and wrap-around services. The service-care coordinator aids clients throughout the GJHA senior housing properties, as well as senior voucher holders living in the community.
